You have had your licence endorsed. In this case please tick the endorsement box and complete your sign up.
You have received a speeding ticket or traffic infringement. In this case please provide the following details of the infringement(s):
Your age
Your gender
Number of tickets/offences within the last 5 years
Description of ticket/offences (e.g. speeding 60km/hr in 50km/hr zone, 20 demerit points)
Description of the penalty for the infringement
Date or approx. date of ticket/offences (ex: May 2019)
